We are a team of highly-trained local professionals and volunteers who provide a medically-directed program of care. Our program is carefully designed to support and comfort patients with a life-limiting illness and their loved ones. Every patient receives expert pain and symptom management to ensure that they are as comfortable as possible.
 
Together with the patient's physician, our team designs a plan of care for our patients, caregivers, and families. We partner with caregivers to regularly address the needs of the patient by providing care in the home, independent living center, assisted living, or long-term care facility. Our services continue in the hospital during authorized medical emergencies.
 
Our professional counselors offer psychological, spiritual, and social services as required by the patient, caregivers, family, and friends. Grace Hospice is dedicated to building a support system that will help everyone involved enjoy life to its fullest potential.

You will never receive a bill.
Any service not covered by Medicare, Medicaid, or private insurance is provided by Grace Hospice free of charge.
 
Grace Hospice provides help and support to you 24 hours a day, 7 days a week.
We are dedicated to your care. Help is available with a simple phone call, day or night. In an emergency situation, we will stay with you up to 24 hours a day.
